I have been working with several emergency physicians over the last 3 days to create plain language and visual based resources for handing to patient at assessment centres and emergency departments. There are 2 sheets, self-management and self-isolation. The sheets are printable editable pdfs at the moment. They are based on current evidence and guidelines.
The self-management sheet can also be used and spread among community groups to get people to stay at home when they feel sick and not show up for testing because the emergency departments are overrun with people with mild symptoms and there are not enough test kits to go round.
I have attached the latest sheets (static versions). The project is here:

We are translating into 15 languages. If anyone would like to help make an accessible version please let me know. Also, please feel free to spread this/use the sheets as you see fit. Some uses so far include for condo buildings, community services, neighbourhood lists, workplaces.
